Athletics
In order to participate in school sports we will need a signed Sports Consent and Concussion Protocol form, as well as a letter from the students Dr. clearing them for sports activity. Students will also need to be academically eligible to play. Sports Forms
In order to participate in sports at Caravel we need a copy of a physical completed from your students Dr. within the last 12 months clearing them for physical activity! (Please note that students will not be allowed to try out for their sport with out an updated physical on file)
We also need the two forms below filled out by you and your student! These forms are often referred to as the "blue" and "green" forms however if you are printing them off to fill out they do not need to be in color!!
